REGISTRATION INSTRUCTIONS - US LACROSSE MEMBERSHIP
1. DO NOT SEND ANYTHING DIRECTLY TO US LACROSSE
ALC registers all participants with US Lacrosse. Turn in the signed US Lacrosse form by mail to ALC, PO Box 16224, Alexandria, VA 22302
2. SUBMIT THIS FORM TO ALC IF ANY ONE OF THE FOLLOWING APPLIES:
1) This is the first time your child is trying lacrosse.
2) Your child is not currently a member of US Lacrosse
3) Your child is a member of US Lacrosse but not an ALC player. Submit the form with your child’s membership number.
3. ALC 2007 Spring Season players are US Lacrosse members
Do not submit this form to ALC if your child participated in the ALC Spring 2007 season. Your child is already a member of US Lacrosse.
